The Forms Forum

Periodically, faculty and staff need copies of the various forms used by district office staff to track payroll.  Use the links at the right to download and print the forms on an as-needed basis.  Note that most of the forms require Acrobat Reader to access. While most machines within the Le Roy Central School District have Acrobat Reader installed, home users may need to download this free utility prior to accessing the forms.
